What is Transitional Care/Swing Bed Services?
Transitional care (swing bed) services are a stop between hospital and home, where patients get the continued care they need while recovering from an illness, injury or surgery, prior to making a safe return to home. A provider will help a patient determine if transitional care is appropriate.
Patients enjoy the comforts of home, while family has the comfort of knowing their loved one is receiving the help they need with on-site care, 24 hours a day. The length of stay varies, but the quality of care is consistent. After receiving care, patients can return home stronger and with more confidence.
What is Offered at Ridgeview
Ridgeview offers a high level of care for patients who are ready to be discharged from the hospital yet require additional assistance before returning home. Ridgeview’s multidisciplinary team includes registered nurses, social workers, certified nursing assistants, pharmacy, providers, and rehabilitation professionals, such as physical, occupational and speech therapists.
Patients can expect:
- Private rooms with tv/cable and phone
- Onsite whirlpool spa
- Option to wear personal clothing for comfort
- Provider and skilled nursing staff onsite 24/7
- Admissions accepted seven days a week
- Weekly care conference including care team, patient and family/caregivers
- Onsite lab and imaging services
- Medical care services including:
- Enteral feeding
- Insulin monitoring
- IV antibiotics or therapy
- Pain management
- Physical, speech and occupational therapy
- PICC/port care
- Post-surgery care
- Wound care

When care goals are met, Ridgeview’s team can make the transition home easier for patients and their families. As patients leave transitional care, our social worker is here to assist. Help may include:
- Arranging home care if needed
- Helping find alternate living arrangements
- Coordinating community services and help
- Providing guidance if a patient needs home medical equipment and supplies
