Bring your completed form to your physician to complete during your preoperative physical exam within three weeks of your surgery date.
Be sure to ask your doctor about any medications you should or should not take the day of your surgery. Ridgeview’s Anesthesia Department also provides recommendations about medications on the Preoperative Anesthesia Form.
