What is a patient portal?
A patient portal is a free, online service that gives you 24/7 access to your health record any time, any where. Ridgeview is using a patient portal product called FollowMyHealth.
What is FollowMyHealth?
FollowMyHealth is the online tool Ridgeview uses for patients to access their health records. FollowMyHealth interfaces with Ridgeview’s electronic health records to securely display your information.
What will I be able to do in FollowMyHealth?
Once you are set up with access to FollowMyHealth, you’ll be able to:
• Review visit summaries from your provider
• Communicate with your clinic by sending and receiving secure online messages
• View test and lab results
• Request prescription refills
• Request clinic appointments
• Receive clinic appointment reminders
Who can create a FollowMyHealth account?
Anyone ages 18 and older who is a Ridgeview patient can access his/her personal health record through FollowMyHealth.
Can I access my children’s records?
Parents/legal guardians can have access to their children’s (ages 0-11) electronic health records through a proxy account in FollowMyHealth. At this time, parents of children ages 12-17 are not able to access their children’s electronic records through FollowMyHealth; state legislation protects children in this age range, and, to uphold these laws, Ridgeview has restricted online access. Parents are able to request copies of their children’s medical records from Ridgeview by completing a medical release form.
Can I access another adult’s records, such as my adult child or spouse?
Adults can grant access to their health record to a parent(s) and/or spouse through a proxy account.
I am the legal guardian/power of attorney for a relative/friend. Can I access his/her records?
Adults who help manage the medical care of another adult, can access the online health record of the patient, if the patient or legal representative of the patient authorizes.
What is a proxy?
A proxy is a person –a parent, legal guardian or power of attorney – who makes health care decisions for another person.
Why should I register with my personal email address?
Ridgeview is requesting your personal email address, rather than a work account, so that if you change employment, you’ll not need to change your email address. In addition, you may prefer that your correspondence regarding your health information be kept in your personal inbox.
Do I have to participate?
No, participation is optional, but Ridgeview does encourage you to engage in your health care by accessing your health record online.
What if I don’t have an email address?
If you don’t have an email address, you can create a free account through a variety of sources, including Gmail, Yahoo, your internet provider, etc.
Is my information secure?
To access any of your health information, a user name and password will be required. The website is secure and adheres to all applicable Ridgeview policies as well as legal standards. Any messages you submit through the portal are secure. Email messages you receive from the portal will not contain private information; rather you’ll be notified that you have a new message within the portal and will need to login to access it.
How do I register for FollowMyHealth?
If you would like to participate, please provide your email address when registering for your next hospital or clinic visit, or contact your Ridgeview Clinic to request an authorization form (note: you will need to visit the clinic in person and present a valid ID).
I’m having trouble logging in for the first time or accessing my account. Who do I contact?

Does Ridgeview offer e-visits?
No, Ridgeview does not offer e-visits. For questions about your clinical care, you can use the messaging feature in FollowMyHealth, but for the diagnosis and/or treatment of a condition, illness or injury, you will need to make a clinic appointment.
Ridgeview does offer Saturday morning appointments at its clinics in Delano and Spring Park, 24/7 emergency care at Two Twelve Medical Center in Chaska and Ridgeview Medical Center in Waconia, and 24/7 urgent care at Two Twelve Medical Center in Chaska for your convenience.
I already have a Ridgeview account. Why do I need to set up a new account for FollowMyHealth?
FollowMyHealth is a new resource available online, through Ridgeview’s website. Any previous accounts you may have created on Ridgeview’s website (such as for event registration, job applications for Family Health Manager) are separate.
Are FollowMyHealth and Family Health Manager the same online tool?
No. FollowMyHealth is the online tool Ridgeview uses for patients to access their health records. Family Health Manager is a separate resource in which you can take a health assessment, create and track health goals, and access local resources and health information; it does not include any medical information from your Ridgeview Clinic or Ridgeview Medical Center visits.
